The vast majority of forms required for internal use in submitting proposals and obtaining IRB approval for protection of human subjects are contained on the Office of Sponsored Programs Administration web-page at http://www.spa.msstate.edu/forms and on the Office of Regulatory Compliance web-page at http://orc.msstate.edu/quicklinks/forms.php. Faculty and research staff should keep in mind that the U.S. Office of Management and Budget (OMB) Circular A-21 requires that certain expenses be treated as indirect costs and paid for through the indirect cost (F & A) mechanism. Costing procedures and definitions of direct and indirect costs can be found at http://www.spa.msstate.edu/quicklinks/cost.php. The OMB A-21 Exception Request form is located at http://www.spa.msstate.edu/quicklinks/pdf/OMBExceptions.pdf.
Sole-source purchases and purchases for research and exemptions from state contract purchases require prior approval; these forms can be found on the Office of Procurement and Contracts web-page at http://www.procurement.msstate.edu/forms/. Sponsors occasionally require a Financial Disclosure Form to ensure that PIs or Co-PIs do not have a conflict of interest in a given research project. The Financial Disclosure Form is used for reporting significant financial interests by sponsored projects investigators and, if required by the sponsor, can be found on the Office of Internal Audit web-page at http://www.msstate.edu/dept/audit/PDF/7009A.pdf. External consultants may be required to complete certain research activities where the PI certifies that no MSU faculty or staff can perform the work. Forms for consulting services can be found on the Human Resources Management web-page at http://www.hrm.msstate.edu/forms/.
